10th Grade Physicals

The State of Connecticut requires a health assessment for students during their tenth grade year.  By state law, the health assessment (physical) must be recorded on the Connecticut Department of Education Health Assessment Record form (HAR-3) form.  The health assessment may be completed by your physician any time from June 1 of the student’s 9th grade year through the last day of school in June of their sophomore year.  The Health Assessment Record form will be accepted as the required physical for tenth graders who wish to participate in athletics.

A student may be exempt from the immunizations requirement of the law and local regulations if the student’s parent/guardian files a written declaration with the school nurse objecting to the immunizations on religious grounds.  A religious exemption certificate may be obtained in the school health office or the administration office of the high school. This statement is required to be acknowledged by a judge, family support magistrate, clerk or deputy clerk or court having a seal, town clerk, justice of the peace, or an attorney admitted to the CT Bar.
