Physical forms are the only forms that are accepted on paper. All other forms must be completed through a PowerSchool Parent Portal account using a computer or laptop, not through the mobile app. Information about Physical forms is available in person from the Athletics Office and the School Health Office or by visiting the School Health Office's website.
